Note Regarding Percent of Increase: This form correctly calculates the Percent of Increase the same as the Department's rent increase database, but you are still responsible for the accuracy and compliance of your notices. Please check your calculations carefully. Increases of more than 2.60% with effective dates in 2012 are eligible for mediation.
